Class A/B:Apply using Form MD‑3/MD‑4, get license in MD‑5/MD‑6 from the State Licensing Authority (SLA).

Class C/D:Apply using Form MD‑7/MD‑8, license granted in MD‑9/MD‑10 by the Central Licensing Authority (CLA) (i.e., CDSCO DCGI Office).

Import

All classes:Import license applied via Form MD‑14, granted in Form MD‑15 by the CLA.

Test license (manufacture):Use Form MD‑12/MD‑13 (A–D) for test batches.

Test license (import):Use Form MD‑16/MD‑17.

Clinical investigation permits:Use Form MD‑22 (apply) → MD‑23 (approve).

For new IVDs or investigational devices: MD‑26 → MD‑27.

Submit a full DMF/PMF, clinical data, QMS (ISO 13485/GMP), labeling, stability and shelf-life proof.

Clinical data: mandatory for C/D, optional or waived for A/B.

Labeling must include manufacturing details, batch, expiry per MDR 2017.

Licenses are perpetual, subject to payment of retention fees every 5 years.
Major changes (e.g., site or constitution) require new applications. Report within 45 days, obtain fresh license within 180 days.
Minor changes need intimation within 30 days.

Registration: Food business operators whose annual turnover is up to 12 lacs fall under the registration criteria of FSSR (Licensing and Registration of food businesses), 2011 under FSS Act, 2006.
State license: Food business operators whose annual turnover is more than 12 lacs and up to 20 crores falls under the state licensing regime of FSSR (Licensing and Registration of food businesses), 2011 under the FSS Act, 2006.
Central license: Food Business Operators whose annual turnover is more than 20 crores falls under the central licensing regime of FSSR (Licensing and Registration of Food Businesses), 2011 under FSS Act, 2006. Central license is mandatory for Proprietary food, Non-specified food, Food Irradiation, Importers, Exporters, E-commerce, Head Office/ Registered Office, Products fall under FSSR (Health Supplements, Nutraceuticals, FSDU, FSMPI, and Prebiotic and Probiotic Food), 2022.
